Step 1. Please complete the ‘Participants Registration Form’ form below.
Step 2. Once completed, please click ‘Submit’ and then click ‘Download form’
Step 3. Please print the completed form and add employer stamp.
Step 4. Scan and upload the stamped form.

Please note that only employers can register participants with APS. It is necessary to include the following documents with the employee’s registration:

• Copy of valid picture ID (passport, ID card, driver’s license)
• Medical Evalutation Letter
• Copy of “benoemingsbesluit” or contract
